Venous embolization, commonly known as venous leak embolization, is a specialized, minimally invasive procedure. It is designed for men whose erectile dysfunction is caused by veno-occlusive dysfunction—a condition where blood escapes too quickly from the penile veins, preventing the maintenance of a firm erection.

Understanding Venous Leak (Veno-Occlusive Dysfunction)

During a normal erection, blood flows into the corpora cavernosa (the spongy erectile tissue of the penis) and the surrounding venous channels compress to trap the blood under pressure. In men with a venous leak, these drainage veins fail to close properly. Even if arterial blood flow is strong, the blood drains out prematurely, making it difficult or impossible to achieve or maintain an erection sufficient for intercourse. This condition often leaves men unresponsive to or dependent on oral medications or penile injections.

How the Procedure Works

Here at AUNC, the procedure is performed by one of our interventional radiologists under local anesthesia, the procedure targets the abnormal pelvic and penile veins draining the erectile tissue:

  1. Cavernosography/Mapping: Contrast material is injected into the corpora cavernosa to visualize the internal blood flow under real-time X-ray and precisely pinpoint where the venous leakage is occurring.
  2. Catheter Navigation: Through a tiny needle puncture (typically in the groin or near the base of the penis), a micro-catheter is threaded through the vascular system to the specific draining veins.
  3. Embolization: The physician deploys specialized medical tools—such as tiny micro-beads, liquid sclerosants—to permanently seal off the leaking pathways.
  4. Restoring Pressure: By closing the faulty outflow tracks, the vascular system can successfully trap blood within the penis to restore natural rigidity.

Ideal Candidates

  • Men diagnosed with venous leak confirmed via specialized imaging like penile Doppler ultrasound
  • Patients who experience partial or complete failure with oral ED medications or penile injections.
  • Individuals who have a preserved arterial blood supply and want to avoid more invasive surgeries or permanent penile implants.

Potential Risks and Side Effects

While generally well-tolerated, venous embolization carries minor and temporary side effects:

  • Post-Procedural Discomfort: Mild to moderate aching in the pelvis, perineum, or injection site, which is easily managed with oral anti-inflammatories or pain relievers.
  • Bruising or Hematoma: Minor discoloration or localized swelling at the vascular access site.
  • Temporary Urinary Symptoms: Transient frequency or urgency as surrounding pelvic tissues adjust to altered venous drainage.
  • Recurrence: In some cases, collateral veins can open over time, meaning the leak may recur, though repeat procedures or alternative therapies remain an option if needed.
